You need to fill a Dematerialisation Request Form (DRF) to convert your physical share certificates into electronic form and hold them in your Alice Blue demat account.
The DRF must be submitted along with your original physical share certificates to complete the dematerialisation process.
Follow the steps below to fill the DRF for Alice Blue:
1. Select the checklist
Tick the option “Normal Dematerialization.
2. Enter the DP ID.
16-digit Demat Account Number
3. Enter the name of the holder(s)
Enter the First Holder’s name exactly as per DP records.
If the account has two holders, enter the Second Holder’s name in the next field.
Both names must match your DP signature records.
4. Enter the Company Details
Fill these exactly as printed on the physical certificate or as seen on NSE/BSE:
Name of the Company
a) ISIN Number
b) Enter the Quantity to be Dematerialised
Enter the number of shares in:
Figures → e.g., 18
Words → e.g., Eighteen only
This must match the certificate.
5. Type of Security (Equity / Preference Shares / Debentures / Bonds)
a) Face Value of Securities
6. Enter the Folio Number
The Folio No. is printed on the physical share certificate.
Enter it as it appears.
7. Enter Certificate Number(s)
Enter the Certificate Number printed on each physical certificate.
If numbers are in sequence, enter the From and To fields.
If not in sequence, enter each certificate number in a separate row.
8. Enter Distinctive Numbers
Enter the Distinctive No. (From – To) as printed on the certificate.
If numbers are continuous, enter the range.
If not continuous, enter each separately in individual rows.
9. Enter the Quantity (per certificate or range)
In the “Quantity” column, enter the number of shares represented by each certificate or distinctive number range.
10. Fill the Holder Details in the Signature Table
At the bottom of the DRF, you will see a table with fields for each holder.
For First Holder – Column 1:
a) Name (as per DP records)
b) Signature with DP
Must match Alice Blue DP records.
c) Signature with RTA
Must match company/RTA signature records.
For Second Holder – Column 2 (if joint account):
a) Name (as per DP records)
b) Signature with DP
Must match DP records.
c) Signature with RTA
Must match the company’s/RTA’s signature records.
If the share certificate is jointly held, both holders must sign in both signature areas.
